Home Support & Community Care

Supporting the independence of older Australians and enabling them to remain in the comfort of their own home is a core belief of ours.

Home Support & Community Care

Whether clients need services such as clinical nursing, domestic assistance, home maintenance, social support, meals on wheels and more, UNITI can support your organisation by providing the tools to ensure efficient service delivery and capturing of appropriate funding data.

UNITI’s simple and effective data collection tools make funding submissions a breeze by producing appropriate funding files ready for upload into the portals. Point of entry data validation and mandatory field highlighting helps ensure you capture all information required to meet your reporting requirements.

UNITI supports a wide range of funding sources, including -

  • Commonwealth Home Support Programme (CHSP)
  • Home and Community Care Program for Younger People (HACC-PYP)
  • Support for Carers Program
  • Department of Veterans’ Affairs (DVA) Community Nursing, Allied Health & Dental
  • National Disability Insurance Scheme (NDIS)
  • Community Health
  • Brokered services
  • Fee-for-service and privately funded

Automate scheduling of client service based on client needs, and easily reallocate client service appointments due to unavailability of staff or clients. View and enter progress/case notes and track the client’s care journey. Schedule client assessments when they are due and complete information with a wide range of standard assessment templates to choose from.

Minimise failure-to-attends through use of SMS appointment reminders and monitor responses, to ensure clients and your business are kept informed of changes in circumstance.

UNITI Mobile enables workers to visit clients in their homes or group settings, whilst still allowing access to client information, past & future services and progress notes – all at their fingertips. Lone Worker keeps your staff safe while in the community, with worker-configured alarms notifying your organisation when service takes longer than expected.

If your organisation has another Patient Admission System (PAS) or Digital Medical Record (DMR), UNITI can integrate with these systems via HL7 and import client data and demographics directly into UNITI. By entering and maintaining client information in one centralised system, this ensures data integrity across both systems. Importing of client data into UNITI also speeds up the client intake process, reducing the need for re-entry and improving efficiency.

As the sector continues to grow and improve, UNITI maintains all changes to appropriate state and government funding requirements. With the new Support at Home changes commencing in July 2025, we are eagerly awaiting the changes to be announced so we can ensure UNITI is updated to comply with the new program requirements.
