Sub-Acute & Non-Admitted Care

Sub-Acute and Non-admitted Care

UNITI can assist with scheduling, delivering and funding your admitted and non-admitted (Tier 2 clinics) sub-acute services such as rehabilitation and palliative care.

Automate scheduling of client service based on client needs, and easily reallocate client service appointments due to unavailability of staff or clients. View and enter progress/case notes and track the client’s care journey. Schedule client assessments when they are due and complete information with a wide range of standard assessment templates to choose from.

If your organisation has another Patient Admission System (PAS) or Digital Medical Record (DMR), UNITI can integrate with these systems via HL7 and import client data and demographics directly into UNITI. By entering and maintaining client information in one centralised system, this ensures data integrity across both systems. Importing of client data into UNITI also speeds up the client intake process, reducing the need for re-entry and improving efficiency.

With majority of sub-acute services in Victoria being reported to the Victorian Integrated Non-Admitted Health Minimum Dataset (VINAH), UNITI has the tools and built-in validations to ensure data is captured accurately and is reported correctly. UNITI supports the full range of programs reported to VINAH, as well as providing supporting AIMS reports. Refer to our VINAH funding page for further information.
